Medical drone delivery: regulatory framework, examples and feasibility in France

Delivering a blood unit to an isolated health centre in a few dozen minutes rather than several hours by road: this is already an operational reality in several countries, documented by studies published in leading medical journals. In France, no large-scale medical drone delivery service is currently operational, but pilot projects are emerging with health institutions and local authorities. Here is what international experience shows, the applicable European regulatory framework, and the feasibility conditions for a project in France.

What international experience shows

The most documented case remains Rwanda, where a nationwide drone delivery service for blood and blood products has been operating since 2016. A 2022 study published in The Lancet Global Health, covering over 12,700 orders delivered between 2017 and 2019, measured a 61% reduction in delivery time compared with road transport and a 67% drop in the expiry rate of blood units at the facilities served — with 43% of orders classed as medical emergencies.

Other countries have since launched comparable trials (Ghana, Malawi, as well as more targeted pilot projects in Europe and North America for transporting biological samples between laboratories). What these deployments have in common: they target routes where ground transport time is a direct risk factor (postpartum haemorrhage, stock-outs in isolated areas), rather than a blanket substitute for ordinary medical transport.

What a French pilot project would require

Unlike Rwanda, France has a dense road network and health facilities generally reachable within an hour, which mechanically reduces the expected benefit of a nationwide medical drone delivery service. The relevant use cases stay more targeted: hard-to-reach islands, isolated mountain areas in winter, or links between a testing lab and a hospital during peak urban traffic, where the drone saves time over the road.

On the technical side, a pilot project requires a drone certified for transporting biological products (packaging, temperature control, traceability), a beyond visual line of sight (BVLOS) link, and close coordination with local air traffic — three requirements far heavier than an ordinary open-category service flight. A feasibility study, carried out with the requesting health facility, usually quantifies the typical route, how often the need arises and the airspace constraints before any operational commitment.

The applicable regulatory framework in Europe

A medical drone delivery service almost always falls under the specific category of the EU regulation, because of the beyond-visual-line-of-sight flight and the likely overflight of populated areas between the pickup and delivery points: a specific operating authorisation issued by the DGAC is required, based on a risk analysis (the SORA method) tailored to each route. This framework is significantly more demanding than that of an ordinary specific-category declaration.

Coordination with U-space — the European system for managing drone traffic in high-density areas — also becomes essential once a regular delivery service operates over a town, to ensure separation from other low-altitude airspace users (notably medical helicopters). These requirements explain why medical drone delivery projects in France remain, for now, at the trial stage rather than deployed as a commercial service.
